Practice Information

• One full time BC/BE Hospice physician needed to join a group in the Salt Lake Valley
• BC/BE Hospice & Palliative fellowship preferred
• Preference given to physician with HMDC certification
• Experience as hospice physician required
• 80% community and 20% hospital work
• Employment position with the Intermountain Medical Group
• Visa sponsorship not available 

Requirements

• Board Certified/ Board Eligible Hospice Care physician
• Hospice experience required
• Unrestricted Utah MD or DO license or the ability to obtain an unrestricted license
• U.S. ACGME Residency
