Practice Information

• Intermountain Healthcare is looking to add a family medicine physician to the Kaysville Creekside Clinic
• Physician will be joining three other family medicine physicians and two advanced practice providers
• Expectation to collaborate with one APP
• MA support available with lab and x-ray on site
• Employment position with the Intermountain Medical Group
• Visa Sponsorship not available

Requirements

• Board Certified/ Board Eligible in family medicine
• Unrestricted Utah MD or DO license or the ability to obtain an unrestricted license
• U.S. ACGME Residency
