Tele-Critical Care Nocturnist

Position Scope

Physician will provide medical intensive care services via the Tele Critical Care (TCC) remote monitoring response center located at the Intermountain Healthcare Supply Chain Center.  Intensivist will work with support center critical care registered nurses and advanced practice clinicians to remotely observe real-time clinical data for all critical care patients across the Intermountain enterprise.  Additional personnel may be added to TCC, to include pharmacists and respiratory therapists.

 

Practice Information

  • Physician Responsibilities
    • Support and help execute the bedside attending physician’s care plan
    • Assist in identifying new patient problems and suggest management solutions or alternative options
    • Directly communicate with the patient’s attending physician on the following:
      • Any concern over imminent patient safety
      • Prior to consulting another physician or group
      • When considering transfer of the patient to another facility
      • Significant patient status change or intervention
    • Do not put bedside nurse in the position of contacting the bedside attending for the above issues. This is the role of the TCC physician and an opportunity to demonstrate effective teamwork and value as an expert in critical care.
    • Using best medical judgment, physicians will assist in preventing patient complications by analyzing clinical data and facilitating implementation of agreed upon best practices based on previously reviewed guidelines/protocols
    • Document clinical care provided according to Intermountain policy
    • Adhere to other “Guidelines and performance expectations for Tele Critical Care (TCC) Physicians” set forth in department specific policy
    • Work with critical care APC in a supervisory role while providing care of TCC patients
    • Facilitate transfers from other Intermountain and non-Intermountain facilities while considering available services, coverage, and load balancing
  • Coverage
    • Physician shall provide in-house “restricted” shift coverage at the TCC. Physician shall not provide on-call coverage for any other hospital while on shift at the TCC. The TCC physician work shifts are:
      • Daytime – 12 hours; currently 0700-1900
      • Nighttime – 12 hours; currently 1900-0700
      • Mid-shift overlap – 12 hours

Position Summary

Total full-time productive hours equal 2040 (170 twelve-hour shifts) per calendar year. Nocturnist position is available for 0.6 to 1.0 FTE. If desirable to the candidate, bedside critical care night and weekend call is available for an additional stipend.
